When employing independent contractors in Yarmouth schools, administrators should use the attached form to clearly describe the services provided unless a more formal contract is utilized. Independent contracts must submit a completed W-9 form (see below). Additionally, a purchase order should be used allowing for the total cost of services to be encumbered.
